Privacy Policy

1. Introduction and Scope

Medicare Clinic Etobicoke ("the Clinic," "we," "us") operates the website etobicoke.medicareclinics.ca. This policy explains how we collect, use, disclose, and protect personal information gathered through this website — its analytics and advertising tracking, and its appointment-request form. It is written to follow the Personal Information Protection and Electronic Documents Act (PIPEDA), the federal private-sector privacy law that applies to this website.

This policy does not cover the clinical health records your doctor creates and keeps about your care once you are a patient at the Clinic. Those are governed separately — see Section 12, "Your Clinical Health Records (PHIPA)," below.

2. What "Personal Information" Means

PIPEDA defines personal information as "information about an identifiable individual" — generally, any information that could identify you, alone or combined with other information, but not the name, title, or business contact details of someone acting in a business capacity. This policy uses that definition throughout.

3. Information We Collect

We collect only what is described below. We do not collect payment/financial information, government ID numbers, or clinical/symptom detail through this website — appointment type is limited to the two checkboxes on the booking form ("Walk-In" / "Family Doctor").

4. Why We Collect It (Identifying Purposes)

Before or at the time we collect information, here is what it is for:

We do not use appointment-request information (your name, email, phone, or message) for marketing, and we do not sell, rent, or trade personal information to anyone.

5. Consent

Submitting the appointment form is itself your consent to our using that information to respond to you, as described in Section 4. For analytics/advertising cookies, this site uses an opt-in consent banner: Google Tag Manager and Google Ads tracking do not load until you click "Accept." If you click "Decline," or close the banner without choosing, those tags are not loaded.

You can withdraw consent at any time by clearing this site's data in your browser (this resets your choice and the banner will show again on your next visit), or by emailing us at the address in Section 13 and asking us to stop.

6. Limiting Collection

We collect only the information listed in Section 3, for the purposes in Section 4 — we do not collect additional fields "just in case," and the booking form does not ask for anything beyond what is needed to respond to an appointment request.

7. Disclosure to Third Parties, Including Cross-Border Transfer

We do not disclose personal information to any other third party, and we do not sell it.

8. Retention

We keep appointment-request information only as long as needed to respond to your request and for the Clinic's ordinary administrative record-keeping, and analytics/advertising data only as long as Google itself retains it under its own policies. We do not currently have a fixed, published retention schedule (e.g. "we delete X after Y months") for this website's data — this is a known gap, stated plainly rather than replaced with an invented number, and is one of the items we recommend Meraj/ the Clinic decide on and formalize.

9. Safeguards

This site is served over HTTPS, so information you submit is encrypted in transit. Appointment-request submissions go to a booking workflow accessible only to the Clinic's own staff and its service provider (Imroz Solutions). We describe only the safeguards that are actually in place — we do not claim certifications, audits, or specific technical controls (e.g. a named encryption-at-rest standard) that haven't been verified.

10. Accuracy

We rely on the information you provide us being accurate. If any information you've given us through the appointment form is wrong or has changed, contact us (Section 13) and we will correct it.

11. Access, Correction, Withdrawing Consent, and Complaints (Individual Access & Challenging Compliance)

12. Your Clinical Health Records (PHIPA)

This policy covers the website only — analytics/tracking and the appointment-request form. It does not cover the clinical records your doctor creates and keeps about your care once you're a patient at the Clinic. Those records are governed separately by Ontario's Personal Health Information Protection Act (PHIPA), under which the Clinic acts as a health information custodian, not by PIPEDA or this policy. Complaints about clinical-record handling go to the Information and Privacy Commissioner of Ontario (IPC)ipc.on.ca — not the federal OPC above. If you have questions about your medical records specifically, please ask clinic staff directly.

13. Accountability — Our Privacy Officer

Under PIPEDA's Accountability principle, this Clinic has designated a named individual who is personally accountable for compliance with this policy. 14. Children's Information

This website is not directed at children, and we do not knowingly collect personal information from children through it.

We may update this policy as our practices change. The "Last updated" date at the top of this page reflects the most recent revision. Material changes will be reflected here before they take effect.
